Community Ties Mission Statement
Community Ties of America, Inc. is dedicated to the development of a compassionate comprehensive community based approach to serving people with special health care needs.


Consulting
Community Ties of America, Inc., provides consultative services to Public Entities such as State and Local Agencies and to Private Provider Organizations.  Consultation specialties include Strategic Planning and Implementation, 1915(c) HCBS Medicaid Waiver Programs, Revenue Maximization of Match Funding Programs, Long Term Care Development, and Quality Improvement/Assurance.


Hawaii RACCP
Community Ties of America, Inc., provides recruitment, review, and licensure/certification recommendations for community care foster family homes and case management agencies to the State of Hawaii Residential Alternatives Community Care Program.  This program serves the aged and disabled in foster family settings on the islands of Oahu, Maui, Kauai, Hawaii, and Molokai/Lanai.


Case Management
Is accomplished through 4 components: 1) Individual Screening & Assessments, 2) Service Supports Planning, 3) Referral Services, and 4) Quality Assurance Reviews.

Case Management services are provided to increase the ability of the recipient to remain in their natural home setting and minimize the need for similar facility based care.

Therapy
Includes Behavioral Analysis services.  These services are provided to individuals enrolled in their State's Home and Community-Based Medicaid Waiver, Medicaid State Plan, and Private Pay individuals.  Recipients receive their services in a community based living setting as prescribed by an approved plan of care.  Each therapy service is provided to increase the function, ability, and well-being of the recipient where they reside.

Quality Improvement
Community Ties of America, Inc., provides quality improvement, provider utilization review, and peer review services to private or public health care service providers.  Community Ties maintains contractual agreements with Board Certified Physicians in all generally accepted medical specialties and a staff of Registered Nurses to assist in our quality improvement initiatives.
